The North Perth - North Huron FHT uses a collaborative practice model that guides the ongoing development of the FHT as physicians and allied health professionals work together to meet the needs of their patients. Much of the success of the Family Health Team is due to the close working relationships with the Family Health Network.

The North Perth Family Health Team has amalgamated with the North Huron Family Health Team, as at April 1, 2021.
